Jackie Dale “Jack” Crooks, 55, of Lee’s Summit, Mo., passed away Oct. 11, 2012, at his home.
Funeral services for Jack will be 2 p.m., Tuesday, Oct. 16, 2012, at Meyers Funeral Chapel, 1600 West Main St., Blue Springs, Mo., 816-229-3276. Visitation one hour prior to the service at the chapel. Burial to be at Lee’s Summit City Cemetery.
