• Ice machine not being kept clean. Mold build-up observed.

Hickory Farms, Independence Center, inspected Oct. 25.

• Hand Washing sinks must be provided. Plumbing system for handwashing was not pumping up water. A temporary hand washing system was put in place and the manager was given until Oct. 28 at noon to fix the sink.

• Waitress was noted handling food contact items with contaminated hands. Waitress placed garnish on drinks with bare hands.
